In a 2020 match-up against the Atlanta Falcons, Cohen sustained an ACL and MCL tear during the third week. This injury caused him to miss the entirety of the 2021 season. Compounding his challenges, as he was prepping for a potential 2022 comeback, he suffered an Achilles tendon rupture on his right leg during an Instagram live workout session. Off the field, Cohen faced personal heartbreak, losing his twin brother, Tyrell, in a tragic incident in 2021.
